This problem iswell illustrated in the study byMangurian et al1 in this issueof JAMAInternalMedicine. Theauthorsused CaliforniaMedicaiddatatoidentifypatientswhowereprescribed anantipsychoticmedication.Theythenassessedwhatpercentage of the patients had some form of glucose screening, a recommendation by the AmericanDiabetes Association2 for persons taking antipsychotic medications, in a yearlong period. Overall, 30.1%of individualswere screened. Itwouldbe fair to pointout that theefficacyofscreening fordiabeteshasnotbeen well established. However, that less than one-third had such screening for a knownadverse effect of antipsychoticmedicationuse suggests opportunities for improvement in integrated health care. Among thosewhohad at least 1 primary care visit duringtheyear, theproportionscreenedwassignificantlyhigher at 35.6% vs 19.8% for thosewho had no primary care visit. To improve care for persons with serious mental illness, it will be necessary to break down the silos that separate the mentalhealthandphysicalhealthcaresystems. Integratedcare (care provided by a team of physical andmental health clinicians)—orat least colocatedcare (careprovidedbyphysical and mental health clinicians in the same place)—offers the promise of improving the physical health of individuals with mental illness, as well as the mental health of those seeking physical health services.

The intra-aortic balloon pump in high-risk percutaneous coronary intervention: is counterpulsation counterproductive?
The rapid increase in the number of percutaneous coronary interventions being performed has led to a corresponding rise in the complexity of lesions attempted and the extent of patient comorbidities permitted. In order to redress the balance in these high-risk subsets, interventionalists are furnished with ever-progressive pharmacotherapy and the continuing evolution of mechanical adjuncts, suc...
متن کاملAbdominal aortic peripheral intervention to facilitate intra-aortic balloon pump support during high risk percutaneous coronary intervention: a case report
BACKGROUND The use of intra-aortic balloon pump (IABP) via the trans-femoral approach has been established for hemodynamic support in patients undergoing high-risk percutaneous coronary intervention (PCI). However, there are various challenges associated with its use, especially in patients with aortoiliac occlusive arterial disease. CASE PRESENTATION We describe a case of high-risk PCI with ...
متن کاملElective intra-aortic balloon counterpulsation during high-risk percutaneous coronary intervention: a randomized controlled trial.
CONTEXT Observational studies have previously reported that elective intra-aortic balloon pump (IABP) insertion may improve outcomes following high-risk percutaneous coronary intervention (PCI). To date, this assertion has not been tested in a randomized trial. OBJECTIVE To determine whether routine intra-aortic balloon counterpulsation before PCI reduces major adverse cardiac and cardiovascu...
متن کاملHigh risk percutaneous coronary intervention and intra-aortic balloon pumps: when is the safety net needed?
The goal of percutaneous coronary intervention (PCI) is to decrease the coronary obstruction in a durable manner without compromising or damaging the myocardium so that patients may obtain the clinical benefits from the procedure, namely potentially improved survival in the setting of acute myocardial infarction interventions and improvement in health status for elective interventions. Interven...
متن کاملPercutaneous circulatory assist devices for high-risk coronary intervention.
A unifying definition of what constitutes high-risk percutaneous coronary intervention remains elusive. This reflects the existence of several recognized patient, anatomic, and procedural characteristics that, when combined, can contribute to elevating risk. The relative inability to withstand the adverse hemodynamic sequelae of dysrhythmia, transient episodes of ischemia-reperfusion injury, or...
متن کاملCombined therapy with percutaneous coronary intervention and percutaneous aortic valvuloplasty under mechanical support for an elderly patient with cardiogenic shock.
Percutaneous aortic valvuloplasty is reportedly a useful tool for the management of critical and severe aortic stenosis with cardiogenic shock. However, early percutaneous coronary intervention for cardiogenic shock is beneficial for elderly patients with acute myocardial infarction. We describe a patient with critical aortic stenosis who presented with severe coronary stenosis of the left main...
